Directions to Clinic:
- Located in Fairhaven Plaza
- Next to PeaceHealth Fairhaven clinic, Subway, & Starbucks
- Take I-5 exit 250 onto Old Fairhaven Pkwy (Turn left from I-5 north & right from I-5 south). Turn right into the Fairhaven Plaza & we are in the northeast corner of the Plaza.
- Take Old Fairhaven Parkway east (from Fairhaven Park & 12th St). Right before you get to the freeway, turn left into Fairhaven Plaza. We are in the northeast corner of the Plaza.
