The initial appointment will include a thorough medical history, screening, and assessment to determine your specific plan of care. Please wear clothing that you are comfortable moving around in.

Before your initial visit, please call your insurance company to verify your outpatient physical therapy benefits. If the insurance company requires an authorization or referral, please bring this information with you. Always bring your insurance information, copay amount, photo ID, and contact numbers for insurance providers. To avoid losing time with your therapist, arrive 15 minutes before your first scheduled appointment time to complete the necessary paperwork.
